Medical Leave Letter

When employees need to take time off from work in order to care for their medical needs or those of their family members, it is professional to send a medical leave letter to employers. This should be a formal letter which can be accompanied by proof of medical need, according to the company’s policies on medical leave for employees. This type of professional communication ensures goodwill between employer and employee and keeps important information on file for the future.

Format and Content

The simplest medical leave letter format should include a brief summary of your upcoming medical procedure and the dates you will need to take your leave, clearly stated in the style of a typical business letter. You should familiarize yourself with your company’s policies on medical leave so that you will be aware if any other information will be expected from you, such as a doctor’s note or confirmation of the medical procedure.

Sample

Here is a medical leave letter sample written by a long-time employee who is scheduled to have her gallbladder removed on a fixed date. She writes to her employers to inform them of her need to take medical leave in order to undergo the surgery and recover from its after effects. She lets her employers know what date she will leave, when she will return, and encloses a note from her doctor as confirmation of the upcoming procedure. She uses a formal business letter style.

Dear Ms. Stuart,

This letter is to inform you of my need to take 2 weeks of medical leave at the end of this month in order to have my gallbladder removed. I will be having my surgery on June 29, 2014. If my procedure and recovery go as well as planned, I will return to the office on July 13, 2014. I appreciate your assistance in this matter as I will need the two weeks off work in order to be fully recovered so that I may be more focused on my work when I come back. Enclosed with this letter, you will find a note from my doctor confirming the date of the procedure.

I would like to extend my assurances that I will strive to complete all projects I am currently working on before my surgery, and put forth as much effort as possible to make up for the lost time once I return to work.

If any clarification is needed in this situation, please do not hesitate to leave me a message via telephone at (555)-555-5555. During my medical leave, I may be contacted via email at [email] and will do my best to send responses in a timely manner. I thank you for your consideration and look forward to your response.

Yours sincerely,
