Parent Permission Letter
A parent permission letter, also known as a parental consent form or a parental consent slip, is a document that may request permission to take temporary responsibility for a child by a teacher, youth worker or other individual who may be entrusted with caring for a youth.
Format and Content
The parent permission letter format may vary based on the requests involved. However, basic requests should always include a full description of the request, terms of liability, the environment and activities that the child will be involved in and other vital information should be stated in a clear manner. Basic contact information of the individual seeking permission should be listed and the parental consent slip may be provided at the bottom of the letter.
Sample
This parent permission letter sample is for a local youth group leader, Karl Burk, at a large church in Dallas, Texas. He is the Head Youth Leader and the youth group is getting ready to put on a fundraising event for a mission trip planned in the fall. This fundraising event involves the youth selling cooked meals for people in the community and the church whom the youth group represents, Texas Lake Baptist Church.
I am writing you to ask your permission to have your child participate in the youth fundraising event that will be held this Thursday night at Texas Lake Baptist Church. Our youth will be preparing and serving meals to the community and church members for a fee. This youth group fundraising event is for a mission trip that the youth will be taking in October of this year. All the money that we raise this Thursday will be going to this mission trip, which will be in Ecuador.
This mission trip will allow our youth group members to impact the lives of the Ecuadorian people and teach our youth valuable skills such as good work ethic, economic awareness and the development of compassion for the less fortunate. This Thursday, our youth will be helping make hot meals. If you do not feel that your child is capable of handling hot food or if you have other concerns about liability policies, please do not hesitate to call me at your earliest convenience. My number is given above and I am available at my office from 9-4 on Monday through Saturday. Please see that I have attached a copy of our church liability policy.
Please know that all of the youth leaders at Texas Lake Baptist Church take our responsibility as youth workers into serious consideration when hosting events such as fundraisers.
